Home and Community Services
G.L.O.M.s Home and Community Services is a program that fosters individuals that have specific needs/behaviors based on their intellectual and developmental disabilities. Our HCS program provides support to our valued clients who are living with their families at home, with host home companions or in living facilities such as group homes. These clients are born with these disabilities and need G.L.O.M.’s care throughout the entirety of their life.
Residential Services
- Supported Home Living: This residential option is supported by trained staff that coordinates and provides services for the individual that chooses to live in their own home or in their family’s home. Supported home living services include meal planning and preparation, transportation, assistance with housekeeping and many more services and support.
- Host Home Companion Care: This residential option provides a home and support services for the individual by a foster care provider, the individual can choose to live in a foster home with a family member or other individual as their foster care provider. GLOM HCS contracts with the foster care provider to assist with daily living skills, personal care, transportation and other activities that allow the individual to live as independently as possible in a safe and supportive environment..
- Group Home Living: This residential option provides HCS residential group homes that are 3 bed homes that are available through the Home and Community Services Medicaid Waiver Program, which is certified by the State. Residential group homes provide a safe comfortable living for up to 3 residence per home. Provided with our group homes are 24/7 support services that assist residence with the activities of daily living. GLOM understands that living in a safe comfortable home is fundamental to the well being of all individuals.

Our HCBS-AMH program provides a wide array of services appropriate to each individual’s needs, enabling them to live and experience an even more successful tenure in their chosen community. Our home and community services are also designed to support long-term recovery for individuals who need adult mental health services and support. We can provide these services in community-based settings such as individual homes, apartments, and small community-based residences.
Adult Mental Health
One’s mental health is as important as their physical health. GLOM aims to not only provide the best care but to also promote the optimal mental health of our clients. GLOMS Home and Community-Based Services – Adult Mental Health (HCBS-AMH) state-wide program provides home and community-based services to adults with serious mental illness and substance abuse disorders who are struggling to get by every day due to their condition.
